As you embark on your personal journey to a new and improved you, you will undoubtedly require information about the various payment options available. We accept money orders, credit cards (Visa, Master Card, Discover, American Express), cashiers checks and cash as methods of payment. Our office also works closely with www.ChaseHealthAdvance.com  should you opt for financing. We are happy to assist you when applying for financing. 

Your initial consultation with Dr. Banda is complimentary (free) for cosmetic procedures. Once you decide to move forward with a treatment or procedure, you will be required to pay in full prior to treatment, unless prior arrangements have been made.  Should you need to cancel or reschedule an appointment, we ask that you kindly provide the office 24 hours notice to avoid a $40 cancellation fee.

INSURANCE

Generally, we do not accept insurance for cosmetic procedures as most insurance carriers will not cover the cost of elective treatments and procedures.  However, there are some exceptions.  We do accept most insurance plans for medically necessary dermatology care as well as for many of our vein treatments, including EVLA.  Benefits paid by insurance companies for select procedures vary greatly from carrier to carrier and plan to plan. Therefore, we make an attempt in advance to determine if insurance coverage exists for your particular treatment or procedure.  Our Patient Care Coordinator is readily available to meet with you and provide you with additional information. You are ultimately responsible for full payment of all treatments and procedures rendered regardless of whether your insurance carrier will cover them. 

SMARTLIPO™ POLICY    

A 10% non-refundable deposit will be collected at the time you schedule your Smartlipo™ procedure. This 10% deposit will be applied toward the total cost of your procedure. The remaining balance will be collected 7 days prior to your procedure date. If you choose to cancel your procedure for any reason, the deposit is still non-refundable.

If you decide to cancel your procedure more than 7 days before your scheduled procedure date, for any reason, we will gladly refund 50% of your payment (excluding the 10% non-refundable deposit).

If you decide to cancel your procedure fewer than 7 days before your scheduled procedure date, the total procedure cost, including the 10% deposit is non-refundable.  Should Dr. Banda cancel your procedure for any reason, you are entitled to a full refund (including your deposit) if you cannot reschedule.  

Finally, should you need to reschedule your procedure date, for any reason, we kindly ask that you provide at least 7 day's notice to avoid a Surgery Cancellation Fee.
